Third Party Administration Rate Sheet
To administer self-funded dental and vision plans, PESC Administrators charges
the following fees:
Monthly Recurring Fees
- Dental third party administration* $4.25/employee/month
- Vision third party administration* 85¢/employee/month
- PPO network access 50¢/employee/month
- Utilization review 50¢/employee/month
* Includes all costs associated with third party administration (dental
and vision claims processing; standard reports; PPO provider directories; recruitment
of new PPO providers in specified areas; and help desk assistance for patients
and dentists or optometrists)
One-Time Fees
- Plastic identification cards $1.50/employee (for
2 cards)
- Initial set-up of Standard Plan Document** $250
- Standard Plan Document booklets $1.00/employee
** Includes compliance with HIPAA requirements
Annual Fees
- Filing of 5500 forms (if requested)*** $750 per 5500
form
*** Employers with self-funded plans must file annual 5500 forms
Dental Plan Open Enrollment Services
- Complimentary directories of participating PPO general dentists and specialists
- Flyers for employees explaining the value of using PPO dentists
- Toothbrushes, toothpaste, and floss cards for open enrollment packets or
sessions
- With advance scheduling, PPO dentists and PESC Administrators employees
are available to attend open enrollment forums or health fairs to explain
new benefits, answer questions, and promote good oral health
